Registration

Community Hospital registration staff is happy to assist you through the admissions process as efficiently as possible. Wheelchairs and motorized scooters are available for visitor and patient use.Please feel free to use the available transport vehicles and do not;hesitate to ask for assistance, if needed.

Registration Desks

Our main registration area is located at the front (south) entrance of the hospital. This area is staffed from 7:30 a.m. to 4:30 p.m. Monday through Friday. The emergency room registration area is at the north side of Community Hospital and is staffed with registration staff from 7 am to 11 p.m. Emergency service is available 24 hours a day, 7 days a week. This entrance is also convenient for laboratory registrations, registrations after 4:30 p.m. on weekdays, weekend registrations, and for use on all holidays. The Rehabilitation Center registration area (cardiology and pulmonary therapy, physical therapy, occupational therapy, speech therapy, and nutritional services) is located inside the Rehabilitation Center entrance on the east side of the hospital.

Parking

Parking and entrance for the Medical Specialists Center including the Community Hospital Orthopedic Clinic registration area is located on the east side of the south parking lot. Signs above the entrances designate both the Medical Specialists Center and the McCook Clinic which is also located on our campus.

What and What Not to Bring

When arriving for registration, please be sure to bring your insurance cards and social security number. If you have a Living Will or Power of Attorney for Health Care, we will be happy to scan it into your permanent record and return it to you immediately. If you will be arriving for surgery, you should be contacted by the surgical education coordinator prior to your scheduled surgery date. Please register at the main registration area inside the north entrance of Community Hospital. Do not bring valuables or jewelry. Do not wear makeup or nail polish. Do bring a list of your medications—both prescription and over the counter.
