Top Performer on Key Quality Measures

Community Hospital was recognized as a Top Performer on Key Quality Measures®” by The Joint Commission, the leading accreditor of health care organizations in the United States in November 2015. The hospital was recognized for its achievement on the measure sets of Surgical Care and Pneumonia. Community Hospital is one of only 1,043 hospitals out of more than 3,300 eligible hospitals in the U.S. to achieve the 2014 Top Performer distinction.

Community Hospital was recognized by The Joint Commission for exemplary performance in using evidence-based clinical processes that are shown to improve care for certain conditions. The ratings are based on an aggregation of accountability measure data reported to The Joint Commission during the 2014 calendar year.

This is the third year Community Hospital has been recognized as a Top Performer. Community Hospital was recognized in 2013 and 2014 for its performance on accountability measure data for Surgical Care. As a Top Performer on Key Quality Measures®, Community Hospital represents the top 31.5 percent of all Joint Commission-accredited hospitals that reported accountability measure performance data for 2014.

Community Hospital and each of the hospitals that were recognized must:

  • Achieve cumulative performance of 95 percent or above across all reported accountability measures;
  • Achieve performance of 95 percent or above on each and every reported accountability measure where there are at least 30 denominator cases; and
  • Have at least one core measure set that has a composite rate of 95 percent or above, and within that measure set, achieve a performance rate of 95 percent or above on all applicable individual accountability measures.

A 95 percent score means a hospital provided an evidence-based practice 95 times out of 100 opportunities. Each accountability measure represents an evidence-based practice.
