Community HealthCare and Hospice

The qualified staff of Community HealthCare and Hospice helps people of all ages creatively face the challenge of illness in their own home. Through individualized services, we work closely with the patient, physician and family to provide patient-centered care. A nurse is available by phone, 24-hours a day to address the needs of any client at any time.  Visits are made on an intermittent basis. A professional staff nurse has primary responsibility for each client, ensuring that each individual's care is personalized and beneficial regardless of the length of care or changes which may occur in the prescribed treatment.

Hospice Care

Hospice Care Services is a philosophy of care for individuals with terminal or life-threatening illnesses, and is intended to provide comfort and relief from pain - physically, emotionally, and spiritually. It is based on the belief that these special people should be able to live their lives to the fullest extent possible. They, along with the members of their families, receive care to preserve and enhance their dignity and their sense of personal fulfillment. Hospice Care is available in homes, nursing homes, and hospitals - wherever it is needed.

Hospice services may include medications, equipment, oxygen, extended hours of care during a crisis period, respite care, and acute stay in hospitals as needed for symptom management.  Hospice is paid for by Medicare and a variety of other insurance benefits.

Home Health

Home Health Services provide an alternative for patients to receive care in the comfort of their own home.  Skilled nurses will provide assessments, catheter care, wound care, lab work, IV therapy, and education along with compassion and individualized care.  Services are provided to home bound patients on an intermittent basis and is paid for by Medicare and a variety of other insurance benefits.

In order to reach these goals, Home Health Care includes:

  • Skilled Nursing

  • Home Health  Aides
  • Physical, Occupational, Speech Therapy
  • Dietary Counseling

You want to live your life your way. This includes living in the comfort of your own home surrounded by memories of a lifetime. Lifeline allows you or your loved one to live at home because help is as close as pressing a button.
