New Portal Offers Patients Access to Their Health Information

McCook, Nebraska—Patients at Community Hospital now have the ability to instantly access their health information online through a secure patient health portal. The RelayHealth portal allows patients to more conveniently view, manage, transmit and download certain medical records such as lab test results, immunizations, procedures and allergies at any hour and wherever internet access is available.

“The RelayHealth portal signifies an important step towards improving patients’ healthcare experiences,” said Carrie Johnson, Project Manager. “This program reflects Community Hospital’s ongoing commitment to advancing technology and providing all of our patients with enhanced access to, and ownership of, their personal health care information.”

Patients with a valid email address can begin to use the secure, web-based tool by registering for a patient portal account during the hospital admission process. Upon discharge the patient will receive an e-mail invitation launching them to the RelayHealth portal to complete their account setup. “Patient portals are a component of the Meaningful Use requirements and we are proud to offer this service to our patients,” Johnson added.

How the process works:

• During hospital registration, patients submit a valid email address to the hospital registrar.

• Upon discharge, the patient will receive an email with a link to the patient portal site.

• The patient will verify demographic information to register on the portal.

• The patient will create a user ID and password.

• The patient will choose and answer three security questions to verify their identity in case of password loss.

• Terms of Use agreement is accepted.

• The patient’s account is activated.
