Home Health/Hospice Social Worker: Part-time position typically working Monday-Friday approximately 30-32 hours/week.
The Home Health and Hospice Social Worker is responsible to deliver varied social work services to Home Health and Hospice patients and their families. To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services and community education, outreach and referral. The Home Health and Hospice Social Worker is an integral part of the Hospice IDG.
Qualifications:
- MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE).
- One year of experience in a health care setting required; 2 years of experience as a social worker preferred.
- Current licensure as a Master Social Worker by the State of Nebraska Department of Health and Human Services required
- Valid drivers license required.
