Care Coordination Manager - McCook Clinic - Full-Time

Care Coordination Manager:  Full-time position typically working Monday – Friday.

The Care Coordination Managers’ primary responsibility is to manage care for the patients of the primary care practice to promote effective education, self-management support, and timely health care delivery. This will include developing and monitoring care management processes, providing support to primary clinical teams, and supporting the initiatives of the contracted ACOs and other quality-based commitments of the practice. It will also include identifying the high acuity patient population and working to ensure care coordination for this patient population. The position may involve some patient triage. This position includes a combination of clinical and administrative responsibilities, encompassing the provision of guidance, mentorship, and training for the care coordination team as well as managing staff schedules.

The Care Coordination Manager will work with the Director and Medical Staff of the practice to develop this position to best serve the needs of the patient panel and the primary care teams.

Qualifications:

Current licensure as a Registered Nurse or Licensed Practical Nurse by the State of Nebraska Department of Health and Human Services Regulatory Board and Licensure required.
Clinical nursing experience of 3+ years and 1+ years of management/supervisory experience required.
